Participation & Liability Acknowledgement I give permission for my child to participate in Arc Broward’s Take Your Child to Work Day event. I understand that my child will be participating in supervised activities on campus and agree to release and hold harmless Arc Broward, its employees, and affiliates from any liability arising from participation, except in cases of gross negligence. Medical Consent: In the event of a medical emergency, I authorize Arc Broward staff to obtain necessary medical treatment for my child and understand that I will be contacted immediately. Behavior Expectations: I understand that my child is expected to follow all Arc Broward rules, staff instructions, and safety guidelines. Disruptive or unsafe behavior may result in dismissal from the program. Photo/Media Release: I grant permission for my child to be photographed or recorded during the event for internal and/or external communications, including social media, unless I indicate otherwise.

I understand that I am responsible for drop-off and pick-up of my child at the designated times and location.
